Stakeholders Across Health Care Raise Concerns About the CY 2027 Medicare Proposals

In the Calendar Year (CY) 2027 Medicare Physician Fee Schedule proposed rule, CMS proposed significant changes to Medicare remote physiologic monitoring (RPM) and remote therapeutic monitoring (RTM). The proposals address how remote monitoring services are delivered, staffed, initiated, and reimbursed.

Across publicly available comments submitted to CMS, hospitals, clinicians, patient organizations, rural health providers, accountable care organizations, and other health care stakeholders raised concerns about the potential impact of the proposed policies on patient access and existing remote monitoring programs.

The Alliance for Connected Care’s “What They’re Saying” resource brings together perspectives from across the broader health care stakeholder community to highlight several consistent themes emerging from the public comments.

“CMS has simply not created a mechanism or requirement for providers to report this data. We call on CMS to implement all OIG recommendations and use the data collected through these changes to inform a more nuanced future policymaking on remote monitoring.”

“If finalized, many RHCs indicated they would no longer be able to provide such services to their patients, and it is also likely to significantly impact future uptake of these needed services in rural communities where additional staffing presents unique challenges that are often alleviated through third-party partnerships.”
National Association of Rural Health Clinics

“Changes of this magnitude beginning only months from now would result in immediate and significant disruption for Medicare beneficiaries.”
